Add/Drop Form

Use this form to change your course schedule. All drops must include the signature of either the faculty member or the department secretary before submitting to the Office of the Registrar.

Address/Name Change Form

Use this form to change your address or name with the Office of the Registrar. Name changes must include a copy of a legal document which shows the new name. Note: Name changes to academic records can only be processed while the student is actively enrolled at Biola.

Appeals Form

Students who wish to appeal a fee or registration issue related to the Office of the Registrar may complete an Appeals Form. The Appeals Form should be mailed, delivered or faxed to the Dean of Academic Records in the Office of the Registrar for consideration.

Arranged Course Permission Form

Use this form when registering for an arranged course, directed/independent studies, or internship (Rosemead Internship excluded). This form requires approval from both a faculty member and the department chair before submitting to the Office of the Registrar.

Audit Application

Use this form when auditing a course at Biola. Please note: Audit forms are accepted after the second week for fall and spring semester courses and after the second day of classes for Interterm and summer courses. A pastoral reference form is required for all auditors. Undergraduate bachelor posted transcripts are required for those interested in auditing graduate level courses (class numbers 500 or above).

Departure Form

Use this form to withdraw (including dropping to "0" units), either temporarily or permanently, from the University. Graduating students should use the Graduation Departure Form.

Enrollment Verification/Graduation Confirmation Form

Use this form to request a verification of enrollment status or a confirmation of graduation letter.

Grade Report (Report Card)/Grade Confirmation Request Form

Use this form if you are interested in receiving a grade report (report card) for a single semester or confirmation of an individual grade. Grade reports and grade confirmations are issued at no charge.

Major Change Form (Undergraduate Only)

Undergraduate students only: Submit this form to make a change to your major. This form requires departmental approvals before submitting completed form. If you are changing your minor please use the "Minor Change Form." Graduate students: To change your program, send a letter requesting a change of program to the Graduate Admissions Office at Biola.

Mid-Semester Progress Report Form

Use this form if you are taking one or more classes at another institution during your last semester at Biola. This form is to be filled out by the professor at the 5th and 10th week of the semester and returned to Biola's Registrar's Office.

Minor Change Form (Undergraduate Only)

Undergraduate students only: Submit this form to make a change to your minor. This form requires departmental approvals before submitting completed form. If you are changing your major please use the "Major Change Form." Graduate students: To make a change to your program, send a letter requesting a change of program to the Graduate Admissions Office at Biola.

Off Campus Programs Registration Form

Application for undergraduate students applying to a semester long off-campus program offered through Biola.

Substitution Request Form

Use this form when applying to have a specific course requirement substituted in place of a different course taken either at Biola University or at another educational institution. This form requires major advisor and department chair approval before submitting to the Office of the Registrar. Substitution Approval Original Form.

Transcript Request Form

This form can be mailed or faxed to the Office of the Registrar when requesting transcripts. Faxed transcript requests must include both the signature of the student and valid credit card information.

Undergraduate Graduation Petition

Undergraduate students must submit this form to the Office of the Registrar prior to graduation. The graduation petition should be submitted two semester before graduation. Petitions submitted in the semester of graduation will result in a fee being assessed to the student. This form requires the signature of the student's major advisor before submitting to the Office of the Registrar. Graduate students: Please schedule an appointment for a graduation check with the Graduate Graduation counselor in the Office of the Registrar.
